Side Effects of the Endolaser Procedure

What are the possible causes of a wry mouth?
In medical terms, a wry mouth generally refers to asymmetric facial muscle movement. The most likely cause is impacted facial nerves. Endolaser is a deep-layer laser treatment, and the heat and depth of application can potentially impact nerves if improperly applied or due to individual differences.

Major causes include:
1.Temporary damage to the facial nerve (most common):
Thermal damage: The Endolaser laser fiber generates heat subcutaneously. If applied too close to nerve branches, the heat can cause temporary “shock” or edema in the nerve fibers (neurapraxia). This disrupts nerve signal transmission, leading to loss of normal muscle control and resulting in a wry mouth and unnatural facial expressions.

Mechanical damage: During the placement and movement of the fiber, there is a possibility of slight contact or compression of nerve branches.

2.Severe localized swelling and compression:
After treatment, local tissues will experience normal inflammatory reactions and edema. If swelling is severe, especially in areas where the nerves travel (such as the cheek or mandibular margin), the enlarged tissue may compress branches of the facial nerve, causing temporary functional abnormalities.

3.Anesthetic Effects:
During local anesthesia, if the anesthetic is injected too deeply or too close to a nerve trunk, the drug may infiltrate the nerve and cause temporary numbness. This effect usually subsides within a few hours, but if the needle itself has caused nerve irritation, recovery may take longer.

4.Individual Anatomical Differences:
In a small number of individuals, the nerve’s course may differ from the average person (anatomical variations), being more superficial. This increases the risk of being affected even with standard procedures.

Notes: In most cases, this is a temporary complication. The facial nerve is highly resilient and can usually heal on its own unless the nerve is severely severed.
